Terms of Service

EROSYN Health LLC(the “Practice”) operates a private functional wellness membership. Membership is offered by application and at the Practice’s sole discretion. Acceptance of these terms is a condition of funding a member balance.

Members fund a balance that is held against the member’s account and drawn against services delivered by the Practice. Funding amounts are between $25 and $5,000 per transaction. Balances roll forward without expiration.

Unused funded balance is refundable in full to the original payment method on written request to members@erosynhealth.com. Refunds are processed within ten business days of request.

The Practice does not provide emergency medical care, controlled substances, or substitute for a primary care physician. Members experiencing a medical emergency must contact 911 or their nearest emergency department.

All charges from the Practice will appear on member statements as “EROSYN HEALTH.” The line item on the receipt reads “EROSYN Health — Membership Funding.”

The Practice maintains member records under standard health-record confidentiality. Records are not released to third parties without written member consent except as required by law.

The Practice’s aggregate liability arising from membership is limited to the unused portion of the member’s balance. The Practice is not liable for indirect or consequential damages.

These terms are governed by the laws of the State of Texas. Any dispute is subject to binding arbitration in Grayson County, Texas.
